If you need assistance with your Home Care Package then you have come to the right place.

Lets Get Support are ready to answer your questions and provide you with care management,  assist you to put your support team in place for you and to make sure that your budget is well utilised with the supports that are a high priority for your home care.

Lets Get Support have partnered with Trilogy Care to be able to provide face to face Home Care Package assistance for case management in the Northern Rivers.

Basic eligibility criteria for government-subsidised aged care:
 

Be 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people) 

If you need assistance with personal care or some everyday tasks at home, this may include cleaning, attending appointments, shopping, lawn and garden, and transport if required.

If you need home modifications or mobility aids so you are safe in your home.

If you need respite for a short time if your carer is away and not able to care for you or 

If you are unable to live safely in your home and ready to move to residential care.

  How to apply for a Home Care Package:

Visit My Aged Care website or give them a call on 1800 200 422. You can complete the application by phone or complete an application online. 

Let My Aged Care know what type of care you need:

  • Help at home
  • Short term accommodation
  • Residential aged care

My Aged Care can organise an assessor to visit you at your home. The assessor will determine the level of care you need.

My Aged Care will send you a letter to advise

  • If you are eligible the services you will receive. 
  • You will be added to the National Priority List until a Home Care Package is assigned to you.
  • When you are allocated a Home Care Package you will receive a referral code.





 
Important terms related to My Aged Care and the process of obtaining government-funded aged care services:
 
My Aged Care:

This is a Federal Government service that helps you find and access government-funded aged care services.
 
ACAT (Aged Care Assessment Team):

This team conducts in-person assessments to determine the level of care you require.
Levels of Care:
Home Care Packages have four levels of care, ranging from level 1 for basic care needs to level 4 for high care needs.
 
Wait List:

Once approved for a Home Care Package, you will be placed in the  queue of applicants. This list is based on their care needs, and packages are assigned to the next person in the queue as they become available.
