NDIS Support Coordinator
About the role
Lets Get Support is seeking an experienced NDIS Support Coordinator to join our team in Lismore, NSW. This part-time role will involve coordinating and facilitating essential support services for our NDIS participants, ensuring they receive the care and assistance they need to live independently and with dignity.

What you’ll be doing

Assessing client needs and creating tailored support plans to meet their individual requirements

Coordinating and scheduling a range of support services such as personal care, domestic assistance, social inclusion activities, and allied health interventions

Liaising with clients, their families, and a network of service providers to ensure seamless, high-quality care
-
Monitoring client progress and adjusting support plans as needed to stay within budgets.

Maintaining detailed case notes and records in line with regulatory requirements
-
Advocating for client needs and rights, and navigating government and community support systems on their behalf
What we’re looking for

Relevant tertiary qualification in Community Services, Disability Support, or a related field
-
Minimum 1 year experience in a NDIS support coordination or case management role is preferred.

Strong understanding of the disability support sector, including relevant legislation and funding requirements.

Excellent communication and interpersonal skills, with the ability to empathise and build trust with vulnerable clients

Demonstrated problem-solving skills and a solutions-focused approach

Proficient in using client management systems and technology to streamline operations

A commitment to promoting client autonomy, wellbeing, and quality of life

Key Tasks

Relevant qualifications, such as a Certificate III in Individual Support or equivalent preferred

Resourcefulness, flexibility, and the ability to work autonomously as well as part of a team

This is a varied and rewarding role, this is a casual position and will involve essential support services for our NDIS participants and aged care recipients, ensuring they receive the care and assistance they need to live independently and with dignity. This role may include being a mentor or assisting a participant to build their independent and life skills both at home and accessing community activities and events. assisting with personal care,meal preparation and domestic tasks at their home.
